Please complete the following before the first visit:

1. Register your billing information here for our direct primary care practice ($85/month).

2. Book a “new patient” appointment.

3. Fill consent forms. An email will be sent from “IntakeQ” with our patient agreement and privacy policies the day of your visit.

4. If you have Medicare, review this form.

5. Submit this medical request form to your doctor to transfer your records to Kahn Health.

6. Lastly, please make sure to check your insurance type. DPC doctors cannot be used as a primary doctor for HMO’s.
