Review our Medicare Form; if you have Medicare, it will be sent to you the day of your visit.
Submit this Medical Request Form to your doctor to transfer your records to Kahn Health.

Submit this medical request form to your doctor to transfer your records to Kahn Health.
Lastly, please make sure to check your insurance type. DPC doctors cannot be used as a primary doctor for HMO’s.
