1. Register your billing information here for our direct primary care practice (see our pricing page for details)

2. Book a “new patient” appointment (if you don’t see a time that works call us)

3. Look out for the Patient Agreement Forms & Privacy Policy Forms the day of your visit; it will be sent via IntakeQ.

4. Review our Medicare Form; if you have Medicare, it will be sent to you the day of your visit.

5. Submit this Medical Request Form to your doctor to transfer your records to Kahn Health.
